Job Title: Case Manager Profession: Case Manager Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (07:00-15:30) Experience: Minimum 3 years inpatient clinical experience, at least 1 year Care Management experience License: RN Certifications: Certified Care Manager Must-Have: - BSN Description: A start on 1/19 or 2/2 is needed. The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs. The role requires attendance at interdisciplinary rounds. You will complete initial assessments. The RN Care Manager will facilitate the development of a plan of care in collaboration with the patient, family, caregiver, and healthcare team. The nurse will communicate consistently with medical teams and patients regarding progress of care needs related to post-discharge care needs, such as home health and inpatient rehab. There will be interaction with post-acute care referral sources to establish a plan.
